As we head into 2026, we’re making a few updates—including moving Tuesday Tips inside our Your Nurse Advocate Now membership (Tier 2–Tier 4). And there’s one important piece we didn’t fully address in our Tuesday Tips announcement: why we choose to stay independent—even though it means our services are private pay.

What “Independent” Really Means (And Why It Matters)
When we say we’re an independent, RN-led advocacy team, we mean:
- We are not employed by a hospital system.
- We are not paid by an insurance company.
- We are not tied to a clinic, facility, or Medicare Advantage plan.
- We don’t have to follow a corporate script or meet productivity quotas.
Our job is to be loyal to you and your family—period.

The Hard Truth: Independence Usually Means Private Pay
We also believe independence shouldn’t mean “no support unless you can afford full 1:1 help.”
That’s why we built Your Nurse Advocate Now—a membership designed to make RN-led guidance more accessible for caregivers who want support, education, and a plan, without jumping straight into private-pay consulting hours.
It’s still independent advocacy (we’re not paid by hospitals or insurance), but it gives families a lower-cost way to get consistent support and avoid expensive mistakes.
We understand that private pay is a real consideration. Many families wish Medicare or insurance covered advocacy support the way it covers a procedure, a test, or a hospital stay.
But here’s the reality: most of what caregivers need help with isn’t a single billable medical service.
It’s the in-between work:
- Making sense of diagnoses, medications, and next steps
- Coordinating appointments and communicating across multiple specialists
- Preparing for doctor visits so you don’t leave with unanswered questions
- Sorting out senior living options and what questions to ask

- Support during a transition in care. Making sure we don’t miss anything or have medication errors. For example going from home to the hospital then to a nursing home and then back home again.
That work is essential—and it’s often exactly what falls through the cracks.
FAQ: Insurance, Medicare, and Private Pay Advocacy
- Is a patient advocate covered by insurance or Medicare?
Long-term care insurance may cover our services on a reimbursement basis. Nurse patient advocates can partner with an entity that can assist with Medicare billing however there are many regulations and rules to consider.
- What’s the difference between a hospital patient advocate and an independent patient advocate?
A hospital nurse case manager, a nurse discharge planner, a transitional care nurse or a nurse case manager with an insurance company can do many things a nurse patient advocate can do but their services are often limited and they must function within the constraints of who they are working for. Their services are often dictated by medical necessity, complexity of diagnosis, and case load. They also may not be available when you need them the most.
- How much does a nurse advocate cost?
We have several levels of care depending on what type of support you need. In general, for 1:1 private consulting it runs about $150.00 per hour. We have created packages as well as our monthly membership to help keep your private pay costs as reasonable as possible while giving you the access and support you need when you need it.
- When should a caregiver hire a nurse advocate?
There are many scenarios that warrant hiring an independent nurse advocate. Adult children often hire us to help manage the care of their aging parent and take on some of the responsibilities of managing their care and offer education and insights on healthcare decisions and challenges.
Aging spouses may use us to assist in managing the care of their aging spouse.
We assist with other tasks such as finding a suitable assisted living or independent apartment.
We attend virtually all kinds of doctor appointments and gain access to our clients “My Chart” so we can keep up to date on all the healthcare conditions and updates.
Lastly we help aging adults and their families put a plan in place to age in place or stay in their home for as long as safely possible. We help arrange care in the home, as well as other services such as home health care, therapy, etc.

Healthcare is complex. Insurance rules are complex. And systems often have priorities that don’t match what a family needs in the moment.
When we’re independent, we can focus on what matters most:
- Safety
- Quality of life

2) We can tell you the truth (even when it’s uncomfortable)
Sometimes families need someone who can say:
- “This discharge plan doesn’t feel safe.”
