Cries for Help:

My husband is an auto mechanic and has a small shop in our very rural community. We are 35 minutes to the closest hospital and a minimum of 50 minutes to a larger medical center. One of my husband’s customers had read our book, “Cracking the Hospice Code” that he proudly displayed in his office. His customer asked him if my advocate business was up and running because she thought she could use our help.

We reached out to her and here was her situation. Her husband was in the hospital. She was told there was nothing more they could do and they were recommending hospice. To her this came out of nowhere. The family was aware of his chronic lung disease but did not understand the severity. The family was not ready for, nor could they comprehend the finality of the situation. They were told hospice or not their loved one was going to be discharged from the hospital because there was nothing more that could be done.

The family decided to take him home. They didn’t know how to care for him, they were told oxygen would be delivered and someone would show them how to use it. They reached out with a cry for help.

One day I received a call from a woman that found our information in a booklet given to her by the Aging and Disability Resource Center. They were above assets for assistance and the suggestion was made for her to go through the book and make some phone calls to see if she could arrange for help on her own.

Her spouse was suffering from Alzheimer’s disease. He was too weak to leave the home. Due to this fact she was also homebound by default. No family close by. She couldn’t get out to shop, run errands, or get him to the doctor for his appointments. She wanted him to get his immunizations but didn’t know how to make that happen. She felt bad asking friends to sit with him while she needed to take care of things out of the home.

We were one of two companies that called her back in 24 hours or less. We heard her cry for help.

A granddaughter reached out to us who lived halfway across the country. Her parents had passed, and she was the only family left for her grandparents. There were no urgent healthcare needs at the time, but she felt helpless and believed she had no control over how she could make sure her grandparents’ healthcare needs were addressed and they were receiving the best care possible. There were some recent healthcare challenges that had come up with her grandma and her grandpa was watching out for her and neglecting his own healthcare needs. We heard her cry for help.

How We Can Help:

First and foremost, we assisted each family to put a plan in place to address all of their needs. Each concern was addressed, and a solution put in place. Often a Third-Party health expert can make headway with an aging loved one that you as a family member cannot. We establish a rapport with the family as health experts and our one job is to provide services to make sure the loved one gets the best care possible while giving the family peace of mind that they are not alone in shouldering all the caregiving and management responsibility.

What happens next? In some cases, we provide 1:1 consulting service before suggesting our Your Nurse Advocate Now Membership program. In other cases, such as the granddaughter story, our program is all that is needed to make sure the healthcare needs are met with their aging loved one.

Our membership program provides you with access to a dedicated nurse patient advocate who will work with you to develop a personalized care plan. We understand that every situation is unique, and we take the time to understand your specific needs and preferences. We believe that this personalization is crucial to achieving a successful outcome and we work tirelessly to ensure that your plan is tailored to your needs. Each family gets a 4M’s assessment. The four Ms include: What Matters Most, Medication, Mentation (Mental Health), and Mobility.

One challenge that many families face is navigating complex healthcare systems. With our membership program, you’ll have a knowledgeable advocate in your back pocket who can guide you through the maze of healthcare options, from insurance coverage to finding the right provider for your loved one.

Another common challenge is unexpected healthcare emergencies. With Your Nurse Advocate Now, you’ll have the peace of mind of knowing that you can reach out to your advocate at a moment’s notice, and they’ll be there to help you through any unexpected bumps in the road.

In addition to these challenges, caregivers often struggle with finding time for self-care. Caring for a loved one can be all-consuming, and it’s easy to put your own needs on the back burner. With our membership program, you’ll have access to a wealth of resources, including a private forum, videos, a weekly newsletter, “Senior Saturday”, and a “Hiring a Caregiver” guide, to help you prioritize self-care and prevent burnout. We also share our “Ultimate Guide to Your Nurse Advocate Resources for Caring for an Aging Loved One.”
