Types of Healthcare Resources that Could Be Part of Your Team; Besides your primary care provider, there are other healthcare professionals that may be a good fit and part of your team.

  1. Home Health Care: Private healthcare resources, such as home health care agencies, can play an essential role in maintaining good health. Home health care services provide healthcare professionals that can help manage chronic conditions, reduce the risk of infections, and promote overall health and well-being. Home health care services are convenient and can provide patients with specialized care, ensuring that their needs are met and that they receive the assistance they need to maintain good health.

If you qualify and have Medicare, your skilled home health care services are covered under your Medicare Part A and no co-pays. Another plus would be having a nurse, an aide, or a therapist come into your home and provide treatments and education while you stay in the comfort of your own home during a recovery phase.

Skilled Home Health Care is only designed to be intermittent. It most likely (with a few exceptions) will provide you with 1-3 visits per week for an average of about 45 minutes. The average length of total time in home health may be 3-6 weeks depending on your needs.

  • Outpatient Services: This could include outpatient therapy for physical, occupational, or speech. You would go to an outpatient setting such as a clinic, hospital or free-standing therapy office. This could help you reduce falls, improve your functioning following a stroke or a traumatic brain injury, or something more simple as working on balance with our aging.

You can usually get more intense therapy in an outpatient setting vs your home. This means you may have quicker results.

You must be able to leave your home and tolerate the trips back and forth to therapy. This outpatient service is covered under your Medicare Part B and you may have a co-pay for each visit depending on any additional Medicare coverage you may have.

  • Health and Wellness Coach: This may be for weight loss, fitness, or for example, a yoga instructor. You get individual attention and have your needs met and addressed personally.
  • Palliative Care: This is also covered under Medicare and some insurances. This service focuses on managing chronic diseases that cannot be cured. They also assist you in advance life planning as your disease progresses. The focus is on symptom management and how to best live with your condition.
  • Nurse Advocate: This is a private pay service that can function for you in many different ways. They would be considered the “Captain” of your team and in charge of pulling all the resources together and making sure all your needs are being met by the healthcare goals you set. An Advocate will work with your doctor and medical team to make sure all the services are coordinated, and you understand your treatment plan. A nurse advocate can provide you with resources to manage your own care, they could come along side you and assist you in staying on top of your care and helping you make informed choices and decisions, or they can be a consultant to you and manage the coordination of all of your care in a “do it for you” type of service. There are many kinds of advocates who specialize in many different areas of healthcare. You want to partner with an advocate that has the knowledge and expertise in the area in which you would like help in.

There are many others to choose from. Other professionals that may come along side you as part of your healthcare team could be:

  2. Chronic Disease Management Coach
  3. Diabetic Educator
  4. Mental Health Coach or Practitioner
